How do I find my plan documents?

It depends on how you enrolled.
If you enrolled through Enrollment Manager, your plan documents will be available there. Simply login using the same credentials you used when you enrolled.
Access Plan Documents in Enrollment Manager
1. Login to Enrollment Manager
2. Click on “Documents”
3. Select the plan document you wish to view
If you enrolled using Employee Navigator
Your plan documents can be found by following the instructions below.
1. Find your member ID card
2. Locate the plan information on the front, in the top right corner
3. Click on this link and enter your plan number in the search bar at the top of this page.
When you search for your plan ID, your plan documents will populate. Your plan information will be contained in these documents.
